From the 2012 Clone Wars assortment, released on the unified Movie Heroes / Clone Wars / Discover The Force cardback. Rex appears here in his Phase II armour with updated season-five deco, including the kill-tally hash marks on his forearms, lower legs, and helmet.

- Firing rocket launcher with projectile
- Galactic Battle Game stand, character card, and die
A kit-bashed figure using the torso and upper arms from the CW02 Phase II Clone Trooper with a new helmet, legs, and paint deco. Companion to the 2011 Hunt for Grievous battle pack, which features Rex in his Phase I animated helmet.
